This initial form collects your name, contact details, safe contact method, referral source, children’s ages, requested services and a brief summary. Please do not include identity numbers, detailed allegations or children’s names at this stage.

How we use the information

To review your request, contact you safely, decide the appropriate next step and coordinate services. A request does not establish an agreed service or confirmed appointment.

Who can access it

Access is restricted to authorised staff. Further professional information sharing will be addressed separately where needed. Confidentiality may be limited where disclosure is legally required or necessary for safeguarding.

Storage and your choices

This online system uses hosted infrastructure supplied through OpenAI Sites and Cloudflare. Hosting location, retention periods, backup arrangements and provider terms must be confirmed by the Foundation before live client use. While setup testing is displayed, enter fictional information only.

Contact us to request access to or correction of your information, or raise a privacy concern. Deletion requests are considered subject to applicable record-keeping obligations.
